State Cross Country Information

2024 State Boys & Girls Cross Country Championships

Fans can purchase tickets here
Tickets cost $9 (plus fees)
NO CASH WILL BE ACCEPTED FOR ADMISSION TO THE MEET.

Admission to DHS Athletic Events

Athletic admission fees: $5
(Free for students with school ID)

Annual passes: $70 per person
Purchase your pass at the DHS office or at any of the upcoming home games/events. Passes grant admission to all regular season home Volleyball, Football, Boys Soccer, Girls Basketball, Boys Basketball, Gymnastics, Wrestling, and Girls Soccer contests except Conference Tournaments, WIAA Tournaments, or charity events.

Athletic Registration Forms

All athletes need to have the following forms on file in order to practice and compete in any sport:

DHS Athletic Permission Form
WIAA Physical Form
The WIAA requires that all student-athletes have a physical every 2 years. All students participating in interscholastic athletics must have this card on file at their school prior to practice or participation. Physical examination taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year. This form MUST be signed by both physician and parent.
WIAA Alternate Year Form
The WIAA requires athletes to have a parent sign this form every year opposite the physical. All students participating in interscholastic athletics must have this card on file at their school prior to practice or participation. This form MUST be signed by a parent.
Upland Hills Health Consent to Treatment & HIPAA Form
Concussion, Head Injury & Sudden Cardiac Arrest Information Sheets
This form does not need to be returned. Please read and sign off on the DHS Athletic Permission Form to certify that you have read and understand all of the information contained in these information sheets.
WIAA Athletic Eligibility Information Bulletin
This form does not need to be returned. Please read and sign off on the DHS Athletic Permission Form to certify that you have read, understand and agree to abide by all of the information contained in this bulletin.

DMS Code of Conduct/Consent & Permission Form

Athletic Fees

All Dodgeville High School Athletes must pay a $45.00 participation fee per sport. $90.00 maximum per family, per season.

Middle School Athletes have a $30.00 participation fee per sport. $60.00 maximum per family, per season.

Maximum Athletics Fee:  

  • High School annual family maximum: $250
  • Middle School annual family maximum: $150
  • Combined HS & MS family maximum:  $110/season & $300/annually

Mineral Point Iowa Grant & Highland Athletes

Mineral Point, Iowa-Grant, and Highland athletes must complete the above WIAA Physical Form or WIAA Alternate Year Form, the Athletic Permission Form, UHH Consent to Treatment & HIPAA Form and pay all fees prior to practice or participation.
